Q&A: Shutter Island ending? Spoiler alert – Don’t answer if you haven’t seen it!?

Question by Katie: Shutter Island ending? Spoiler alert – Don’t answer if you haven’t seen it!?
In the film, at the end when you begin to realise he’s insane and has made it all up… a) Has he actually made it up? Is he really insane? b) Or are they tricking him to think that, and he’s actually sane?
a) All of it fits together, the anagrams of the names, the doctors being real people, what his wife did and what he saw at Dachau – did that tip him over the edge?
b) All the people who told him (the ‘real’ Rachel – the doctor in the cave) to get away, that they would get him. So, which one is real? I’ve seen it twice now and am beginning to think he actually was insane but the first time I thought they were tricking him. I can see it from both sides (which I guess is what makes it such a good movie!)
So, what are your thoughts/do you know for sure?
